I'm trying to figure out how someone supposedly died on the way to the hospital. Medics don't call codes in the back of an ambulance. They either call them on scene or transport a working code to the ER.

I have had medics declared time of death in the back of the ambulance a couple of times while I was on a call. Depending on the severity of the injury it may be unfortunately impossible to do successful CPR.

Why do they take patients to hospitals so far away?
